SPECIFIC AIRLINE REQUIREMENTS

  1. Alaska Airlines: 1-800-426-0333
    • 24 to 48 hour advance notice
    • $50 oxygen fee per segment (coupon)
    • Doctor's letter to include:
      • Passenger's need for in-flight oxygen
      • Statement that passenger's health is stable and he/she is able to fly
      • Oxygen flow rate
    • Adjustable flow meter 2 L/M to 8 L/M
    • Choice of nasal cannula or mask
    • Passenger is responsible for adjusting flow rate valve
    • Passenger may send EMPTY portable oxygen through baggage
    • No specific seat assignment for passenger using in-flight oxygen

  2. American Airlines: 1-800-433-7300
    • 48 hour advanced notice
    • $75 per segment (coupon) non-refundable
    • Doctor's letter to include:
      • Liter flow per minute (2 L/M or 4 L/M)
      • Oxygen delivery continuous or as needed
      • Oxygen arrangements are made by the Special Assistance coordinator after flight is scheduled
    • Non-adjustable flow meter delivering either 2 L/M or 4 L/M
    • Nasal cannula or mask
    • Passenger may bring EMPTY portable onboard or send EMPTY portable through baggage.
    • Payment for oxygen can be made ninety minutes before boarding time at the ticket desk or several days in advance at a ticket office

  3. Continental Airlines: 1-800-525-0280
    • 48 hours advance notice
    • MedLink makes the oxygen arrangement 1-800-786-6006 for the airlines
    • Doctors letter to include, liter flow per minute required at 8000 feet altitude and amount of oxygen required per hour (stated in liters per hour), dates of travel, and letter dated 10 or less days from outbound date. Letter must be on doctor's letterhead. Your doctor will be phoned for a verbal approval
    • $50 fee per couple/segment
    • Adjustable flow meter ¼ liter to 6 liters
    • Nasal cannula
    • Passenger's EMPTY portable can be sent through baggage

  4. Delta Airlines: 1-800-221-1212
    • 48 hour advance notice
    • $25 per unit of oxygen with a $50 minimum, non-refundable
    • Airline will provide a maximum of 4 oxygen cylinders per coupon. The cylinders must be stowed under the seat in front of you. If you require more cylinders you may pay for an additional seat next to you or travel with a kind companion
    • Doctor's name, phone number and fax number and your diagnosis must be given to the airline. Airline contacts doctor and determines liter flow per minute and number of "bottles of oxygen" needed. Passenger will then be called and oxygen costs will be quoted
    • Adjustable flow meter 2 L/M to 8 L/M
    • Nasal cannula or mask
    • Window seat only
    • Arrive at least two hours early to check in
    • EMPTY oxygen portable may be checked though baggage

  5. Hawaiian Air: 1-800-367-5320
    • 48 hour advance notice
    • $75 per segment (coupon)
    • Doctor's letter to include: Verification of need and liter flow per minute
    • Adjustable flow meter ranging from 2 L/M to 8 L/M
    • Mask or cannula provided
    • Special Seating required in aircraft
    • Passenger may check EMPTY portable in baggage or take onboard

  6. Northwest Airlines: 1-800-225-2525
    • 48 hour advance notice
    • $75 per segment (coupon)
    • Doctor's letter or prescription to verify passenger's need for oxygen and flow rate per minute
    • Adjustable flow meter from 2 L/M to 8 L/M
    • Nasal cannula or mask provided
    • Passenger may check EMPTY portable through baggage

  7. TWA: 1-800-221-2000
    • 48 hour advance notice
    • $75 oxygen fee per coupon, non-refundable
    • Airlines need doctor's name and phone number. TWA's medical desk will contact your doctor. However, Airline recommends you carry an oxygen prescription with you
    • Non-adjustable flow meter, either 2 L/M or 4 L/M
    • Mask only is provided but you can bring your own nasal cannula
    • Passenger may check EMPTY portable oxygen unit through baggage

  8. United Airlines: 1-800-241-6522 (except Illinois)
    • 48 hour advance notice for domestic flights
    • 7 days notice for international flights
    • $75 oxygen fee per coupon, non-refundable
    • At the time you make your reservation you must give your doctor's name and phone number. Medical desk will call your doctor to confirm need, flow rate, continuous flow or as needed. The medical department will call you to confirm your oxygen arrangements
    • Adjustable flow meter from 1 L/M to 8 L/M
    • Special seating assignments
    • Passenger may check EMPTY oxygen unit through baggage or bring onboard

  9. U.S. Air: 1-800-428-4322
    • 48 hours advance notice
    • $50 per unit of oxygen used, non-refundable
    • Flow meter set for 2 L/M, 4 L/M or 6 L/M
    • Provide airlines with Doctor's name and fax number. Authorization form will be faxed to doctor to fill out. Passenger will pick up form from Doctor and pay for oxygen at the ticket counter at least one hour before departure
    • Passenger must be able to be with out oxygen for at least 15 minutes during boarding and deplaning because airline does not allow oxygen service on jet way
    • Nasal cannula or mask is provided
    • Passenger may check empty portable oxygen unit through baggage
    • U.S. Express does not provide oxygen service

    Please be aware that not all airlines provide oxygen service; i.e., Southwest, Reno air, America West. Also express service from providing airlines does not provide oxygen; i.e., American Eagle, United Express, and U.S. Express. Only jet flights provide in flight oxygen.

    BEFORE THE PLANE LEAVES THE GATE, BE SURE THAT THE OXYGEN EQUIPMENT IS FULL AND THE CORRECT LITER FLOW PER MINUTE IS SET.
